CA Healthy Kid Survey
Dear Hughes Families,
To support student well-being and development, Santa Clara Unified School District (SCUSD) students in grades 5, 7, 9 and 11 will take part in the California Healthy Kids Survey (CHKS) from December 1 to 12, 2025. This survey helps the district understand student needs in areas like connectedness, physical and mental health, and support systems.
The CHKS is an anonymous and confidential survey that assesses school climate and safety, student wellness, and youth resiliency among students in grades 5, 7, 9, and 11. Parental consent is required for students in elementary school, while for secondary students, parents must submit a written request to opt out.
Please click on the links below to review the survey samples your child will participate in, as well as the consent form. Additional information and parent permission forms for elementary students are scheduled to be sent the week of November 10, 2025. The survey for students is for 5th grade ONLY.
In addition, ALL parents/guardians (grades TK to 5th) are invited to participate in the California School Parent Survey (CSPS). Thank you in advance for your participation in this important survey.
